Below are the midwifery program registration materials. You must already be a matriculated midwifery program student to use these forms (If you're not a midwifery student, go to Upcoming Classes for Community Registration materials.) To register, print the forms, fill them out and return them to the school. If you need other forms, please contact us so we can get them to you!

If you would like to register for Research Project, please print out all of the following forms. A completed Registration Form for Research Project must be submitted to Birthingway by the first day of the term, otherwise you will be automatically dropped from Research Project.

Clinical Training Credit Forms

If this is your first time working with a midwife, and you have not attended our Clinical Training Workshop, please meet with our Faculty Coordinator to discuss your clinical training. You must either attend the workshop or meet with the Faculty and Student Affairs Coordinators before beginning your clinical training. In addition, you must notify the school of your intent to begin clinical training by completing and submitting the “Intent to Work with a Preceptor” form to the Faculty Coordinator. The preceptor must be approved by Birthingway before you can begin working together.

You need to complete the following forms every term that you register for Clinical Training Credit:

Preceptor/Student Contract:

Spring 2010 Preceptor Student Contract (86KB PDF)

You must turn in the completed Preceptor/Student Contract and full payment for the credit(s) before you will be registered. Any work you complete prior to submitting the contract and payment will not count toward your total hours. If you need the credit to count toward your financial aid, this form must be completed and turned in by the second Friday of the term.

Clinical Training Timesheet (48KB PDF) - You are responsible for entering the hours you have worked. At the end of the term, the preceptor must sign off on the time sheets, and then you must turn in the timesheets, showing completion of the minimum number of hours to receive credit.

“Evaluation of Preceptor by Student” form (32KB PDF) - This must be completed and turned in at the end of the term to receive credit.

“Evaluation of Student by Preceptor” form (18KB PDF) - (evaluation of student by preceptor) This must be completed by your preceptor, and you must turn in by the end of the term to receive credit.

Skills Assessment Checklists - This is part of your Client Care Documentation binder. If you do not have this binder, you should purchase one (inquire in the Front Office). Your preceptor will review the Skills Assessment checklists and update them with dates and preceptor initials. You must fax or turn in a copy of the checklists at the end of the term to receive credit.
